Walk-In Clinic

Monday – Friday 7:30am to 9:00am
(The Walk-In Clinic is intended for our ESTABLISHED patients with MINOR illnesses (illnesses with symptoms less than 72 hours). There may be a wait, and we can only deal with your acute issue.  This is not the time to treat or discuss chronic issues. All other issues will need to be addressed at a scheduled future appointment.)

We have onsite lab which allows us to offer the collection and processing of most lab tests.  Also, we have the capabilities to provide basic radiology services.  Other services that FCFP provides are, DOT Physicals, Ear Irrigation, EKG, Pulse Oximetry, Allergy Shots, Blood Pressure Checks, Drug Screen Collection, Laceration Repair, Excision of Ingrown Toenails, Well Child Visits, Immunizations, Destruction of Lesions and many more!

Insurance and Payment

At Falls City Family Practice, P.C., our goal is to make the billing and insurance process as easy as possible.  We accept a wide variety of medical insurance, Medicare and Medicaid to help cover your costs.  If you do not have insurance, please contact our Office Manager to discuss payment plans.

We will file insurance claims to insurance carriers that we participate with.  It is your responsibility to assure that we have the correct insurance information on file.  This includes providing us with your current insurance carrier card.  If your insurance has changed, or you have received an updated card, it is your responsibility to present that card to the front desk.  This will allow us to have the most current insurance information, and eliminate delays in processing your claims.

Fees for office visits will vary depending upon that type of service provided.  There are five levels of service for each new and established patient.  The practitioner will decide the level of care they provided during your visit per American Medical Association guidelines.  A new patient is one who has not received any professional services within the last three years.

Co-payments are required at the time of visit or you will be asked to reschedule.

Billing of charges is not final at time of service.

For your convenience, we accept cash, check, money orders, debit card, MasterCard, VISA or Discover

If billing is necessary, a statement will be mailed to you that are due within 30 days.

For Worker’s Compensation claims , it is our policy to bill your employer or the worker’s compensation carrier for services rendered.  It is your responsibility to contact us with the name and address of your employer or the insurance company, which covers your employer.  If you are covered, we will accept the payment made by worker’s compensation as payment in full.  If worker’s compensation denies payment, the entire balance will become your responsibility.

For Motor Vehicle Accident claims , it is the patient and/or guarantor responsibility to inform FCFP prior to each visit when services are related to a motor vehicle accident. MVA insurance billing information is required at the time of the first visit.  We will bill them as a courtesy; however, payment is your responsibility should the MVA insurance not pay.

For prescription refills , please call your pharmacy at least 48 hours in advance, Monday through Friday.  The pharmacy will call us for verification.  Please be advised that it may take up to 72 hours to process your refill request.

P.I.N. Program (Patient In Need Program) is a service we provide to our patients at no cost.  This program is designed to help those in need (based on income guidelines) obtain their medicines at no cost or very low cost.
